Regulations for donations

Donations Regulations

1.
GENERAL INFORMATION

  1. The www.jim.org/polskananiebiesko/ website is operated by the JiM Foundation (hereinafter referred to as the Foundation), based in Lodz, 105 Tatrzanska Street, registered in the Register of Associations, Other Social and Professional Organizations, Foundations and Independent Public Health Care Institutions, kept by the District Court for Lodz-Śródmieście in Lodz, XX Economic Department of the National Court Register under KRS number: 0000127075, PLN, NIP: 7282455613.
  2. These terms and conditions are intended to define the conditions for making online donations using the form available on the polskananiebiesko.pl website implemented by the PayU SA service and the conditions for receiving a gift in the form of a fabric wristband as a thank you for the donation made.
  3. Online payments are handled by the payment operator PayU SA, 182 Grunwaldzka Street, 60-166 Poznań NIP 779-23-08-495, REGON 300523444, District Court Poznań - Nowe Miasto and Wilda in Poznań, VIII Economic Department of the National Court Register, KRS 0000274399

DONATION RULES

  1. Donations can be made by any adult individual, legal entity or organizational unit. These persons will be referred to hereafter as "Donors".
  2. PayU service allows you to make a donation to the JiM Foundation to support its statutory activities.
  3. Donations made via www.jim.org/polskananiebiesko/ can be made from February 1, 2021 to April 9, 2021.
  4. In order to make a one-time donation through the PayU Service, after filling out the form present at www.jim.org/polskananiebiesko, the Donor is duly redirected to payu.pl.
  5. Within the framework of online payments, the Donor can make contributions by:
    - electronic transfers (including BLIK, Google Pay) and traditional transfers (the current list of cooperating banks and PayU SA payment methods is available at http://www.payu.pl/płatności-online) and payment cards.
  6. In addition, from the PayU website, the Donor has the option to download a print for donation deposit at the post office or bank.
  7. Each Donor who fills out the form and goes to PayU service agrees to process the data provided in the form for marketing purposes, in accordance with §4 of these Regulations.

RULES FOR RECEIVING A GIFT

  1. A gift of a poster will be given to a Donor who makes a donation of at least PLN 67.00 with a properly completed form at www.jim.org/polskananiebiesko.
  2. The deadline for shipping the gift is 3 days from the date of the donation.
  3. Gift shipments are made through the Polish Postal Service.
  4. Shipping of the gift is carried out only within Poland.
  5. The poster gift will be awarded from February 1, 2021 to April 9, 2021 or until stocks last.

DATA PROCESSING

  1. By filling in the data in the Form, the Donor accepts the Regulations and consents to the processing of Personal Data by the Foundation, in accordance with the terms of the Regulations and for the purposes specified therein.
  2. The administrator of the personal data is the JIM Foundation, based in Lodz (93-279) at 105 Tatrzanska Street.
  3. The donor has the right to access the data, correct it, demand its deletion, as well as the right to restrict processing, object to the processing of personal data and the right to lodge a complaint with the supervisory authority if the data is processed in violation of legal requirements.
  4. The data will be processed in order to conclude and perform the contract concluded with the Administrator and to make the necessary settlements in connection with its conclusion (where necessary), as well as to fulfill our statutory activities . Data will also be processed for marketing purposes.
  5. Data may be processed in a fully automated manner, including profiling.
  6. Provision of data is voluntary, but the consequence of failure to do so will be the inability to conclude a contract.
  7. To the extent that your data is processed on the basis of a separate consent, you have the right to withdraw your consent at any time. However, this will not affect the lawfulness of the processing that was carried out before the revocation.
  8. Personal data will generally be processed for a period of time until the expiration of the statute of limitations for claims and obligations under the contract concluded with the Administrator.

5.
COMPLAINT PROCEDURE

  1. In justified cases, the Donor may file a complaint and request a refund of the donation made.
  2. Complaint should be reported to: darczyncy@jim.org writing "complaint" in the title of the message and providing the reason and details of the transaction. Each complaint will be considered individually by the JiM Foundation within 14 days of receipt of the complaint.

6.
FINAL PROVISIONS

  1. JiM Foundation has the right to change the regulations at any time.
  2. In matters not covered by these regulations, the provisions of generally applicable law shall apply.
