Pursuant to Article 81 (1) of the Act of February 4, 1994 on Copyright and Related Rights (Journal of Laws of 2019, item 1231, as amended), I hereby declare that I consent to the JiM Foundation, based in Lodz, Poland, recording and providing, free of charge, my image recorded in the form of audiovisual material constituting a several-minute spot for the purpose of casting for the Project, i.e. the mini-series realized by director Damian Kocur.

The transfer of this authorization and any rights related thereto to a third person or entity does not require my prior consent to such action and shall be free of charge.

At the same time, I declare that this consent is not limited in time or territory.

The spot, containing my image, may be used for various forms of electronic image processing, reproduction, framing and composition, without obligation to accept the final product, but not in forms that are offensive or generally considered unethical, and only for the purpose of casting for the Project.

At the same time, I was informed that further work on the Project is paid and carried out with due diligence and health and safety, under the conditions agreed with the Project implementer.
